Here at Reclaim Mobility, as well as working with individuals who choose to have therapy, we also work with Case Managers to provide a service over Essex where we specialise in providing comprehensive therapy services for individuals who are under Case Management.
Whether it’s clients’ who have experienced catastrophic injuries, road traffic accidents, traumatic brain injuries, spinal cord injuries, and complex neurology cases, we understand the challenges faced by both the clients and their case managers during these difficult times.
Due to the unique multi-disciplinary therapy team that we have here at Reclaim Mobility, we are in a special position to support your clients’ with their complete therapy needs through one team without having to outsource and liaise with different
services. Our team of experienced and skilled therapists of physiotherapy, occupational therapy, speech and language therapy and clinical psychology is dedicated to supporting and working closely with case managers to ensure your client gets the best therapy with streamlined, effective team communication.
Without the need to discuss your client’s needs repeatedly to different professionals, you can liaise with just one person, our Clinical Director, who will ensure appropriate team members are handed over the relevant information for you. As all our team
share resources/systems, it means there is full transparency with communication between all our therapy staff.
As part of our MDT approach to case management cases, we are keen to incorporate regular team meetings with all involved to ensure complex cases are managed to a high standard and that the service we provide is of an excellent quality.
Client’s have the benefit of knowing they are under one team for their therapy making it easier for them to know who to contact if they have any questions or queries.
How we work with case managers:
At Reclaim Mobility, we believe in a collaborative approach to care. We work closely with case managers to develop personalised treatment plans tailored to the unique needs and goals of each client. By maintaining open communication channels, we ensure that case managers are involved every step of the way, allowing for seamless coordination of care.
We understand that every client is different, and therefore, we take a holistic approach to rehabilitation. Our multidisciplinary team conducts thorough assessments to identify specific impairments and challenges. As we have a broad team of therapy professions, this allows us to design targeted interventions that address physical, cognitive, and emotional needs.
We serve as advocates for our clients, empowering them to voice their needs and concerns. Our team works to ensure that clients receive the resources and support they require to maximise their potential for recovery and independence. We also offer guidance and support to case managers, helping them navigate a client’s complex health needs and can provide recommendations as requested i.e. equipment recommendations.
Our Specialised Services:
Our experienced physiotherapists utilise evidence-based techniques to improve mobility, strength, and function following catastrophic injuries and trauma. Through personalised exercise programs, manual therapy, and assistive devices, we help clients regain independence and enhance their quality of life.
Our occupational therapists focus on enhancing clients’ ability to perform daily activities and participate in meaningful occupations. From adaptive equipment and home modifications to cognitive retraining and vocational rehabilitation, we address barriers to independence and promote successful community reintegration.
Our speech and language therapists specialise in addressing communication, swallowing, and cognitive deficits resulting from traumatic brain injuries and neurological conditions. Through individualised therapy sessions and augmentative communication strategies, we support clients in rebuilding communication skills and regaining autonomy.
Our clinical psychologists provide expert assessment and psychological interventions to support clients’ emotional well-being and adjustment to life-altering injuries. From coping strategies and stress management techniques to trauma-focused therapy, we help clients navigate the psychological challenges associated with catastrophic injuries and trauma recovery.
Our experienced assistants can practise exercises and individualised therapy plans with clients to ensure continuity of therapy, between therapist visits, as well as allowing a more cost effective service for case managers if this is needed.
Working with other health and social care teams
As well as working with Case Managers, due to our years of experience working within the local authority and the community based teams connected to that such as Collaborative Care, Reablement, Early Supported Discharge Teams etc, we are experienced and able to support with care based assessments also. This could be you or your client are aiming to reduce their care package and want to know if they are safe to reduce their care provision or you want to progress your client’s abilities during your visits, then we can help by doing joint visits with other professionals/carers providing evidence to go towards care reviews and risk assessments or in any other way deemed appropriate.
To ensure a seamless approach to your health and social care needs, we encourage up to inform any other health and social care teams that you are undergoing therapy with us providing them with our contact details. This way, we can update professionals on how you may be doing (with your consent of course!) to ensure that you get the best possible services and input all round.
